This waiver mandatory for online registration

1. RELEASE of LIABILITY - I agree to release Michelle Denny and Dance Discovery of Austin LLC, its instructors and assistants, Abacus School of Austin, The Children’s Center of Austin, Stepping Stone Schools, Four Seasons, Bright Horizons, The American Legion Post 76, and Mark Cashman from any liabilities including accidents, injuries or other occurrences. 

2. PHOTOGRAPHY AND VIDEO - Dance Discovery has my permission to photograph and videotape my child during classes and performances. I agree that these images of my child may be used for ongoing public advertisements.    

3. SCHOOLS & CENTERS -I understand that if my child takes a class on a day that s/he attends Abacus School, Four Seasons, The Children’s Center, Stepping Stone, or Bright Horizons, s/he will need to arrive to school in dance attire. Students will not be taken to dance without the proper attire.

4.  INSTALLMENT PLAN - I also authorize Dance Discovery to automatically charge my account for my monthly payment on the first of each month, as well as a costume fee for each class enrolled and a recital fee per child for the Spring Recital. (Toddler, Yoganastics, and Lunch at the Theatre classes will not require a recital fee because their performance will be in the studio.) A $30 service fee may be applied to my account when it is no longer in the automated system due to a declined, unfunded, or rejected payment.

5. REFUNDS AND DROPS - You must give us 30 days notice if you wish to drop a class.  A student may only drop a class by calling the Central Studio office at 419-7611. Dance Discovery teachers can not edit payments. Daycare centers and preschools do not provide withdrawal information to our staff, parents must do this themselves. Drops may not be made for the months of May and August as these are culminating months for our programs. If I drop a weekly class, I understand that I am responsible for paying tuition for the 30 days after notification. If I owe a tuition balance, I will be notified of the final payment via e-mail before it is processed. After the first class, all tuition and fees are non-refundable once paid. If I drop after the second class the first tuition installment, registration fee, costume fee, and recital fee are nonrefundable. Camps are nonrefundable, a $15 processing fee will be charged to transfer to another camp/class with openings.

6. DISEASE CONTROL- I understand the children may not attend class within 24 hours of fever or vomiting and all open sores must be covered completely.

7. POLICIES -I understand that Dance Discovery has the right to change these policies at any time and I will be emailed policy change notifications.
